Feedback on the JFS ICCA form 3/17/16 Page 1 Types of placement: • We would like Emergency shelter care added as an option. • We would like psychiatric hospital deleted as an option”this would not be a placement, but a leave. • We would request that Relative/Kinship Home be changed to Kin/Kinship to align with rule language. Section2 We recommend removing dependency from the below statement “Describe history of abuse and/or neglect and/or dependency (both past and current, identify intergenerational influences, if applicable.” OAC states that a history of maltreatment should be included and dependency is not maltreatment by definition. Page 2 Development issues: These fall under characteristics that should be entered into SACWIS. If this form is in SACWIS and pulling from SACWIS we recommend the ability to pull the characteristics from SACWIS for the Characteristics addendum they are recommending. We also would request that the ability to add new characteristics while completing the ICCA be an option. As the Development section appears it looks like information would be duplicated. Physical Health Care Issues: Under the medication section it would be helpful if there were prompts as to what the medication was, what is was for, how often and at what dosage should it be taken. All of this information goes into the person profile section of SACWIS. We would recommend that once this information is entered in the ICCA it would be pulled into the person profile section if it was not already there. And if the information was already in SACWIS it would pull into the ICCA. Mental health issues: We recommend results of testing be changed to Diagnosis. All of this information goes into the person profile section of SACWIS. We would recommend that once this information is entered in the ICCA it would be pulled into the person profile section if it was not already there. And if the information was already in SACWIS it would pull into the ICCA. Page 3 Section 4: First sentence: “Has the child committed other violent acts or exhibited behaviors of concern?” We recommend removing the word other as it is not needed. Section 4 also mentions “and PCSA of the county in which the child is being placed (if the child bas been adjudicated delinquent and/or unruly at any time within the placing county). The PCSA is to be provided information about the child per Rule 5101:2-42-90.” This is not in the above mentioned rule as a requirement and we ask that sentence be removed and the check box below it also be removed. There is also a location for initials in this section. Since the caseworker is signing the document we do not see a need for other initials. Section 5: We recommend completely revising this section. We recommend the section have the following questions: Is this an initial placement? Removal reason (If it is abuse/neglect include who the AP is) Describe the circumstances or youth’s behaviors that led to a need for placement or placement move? Explain why the risk of harm to the child was greater than the emotional trauma and other harm potentially caused by removal of the child from the home? State the reasonable efforts which have been made to prevent the removal and/or make it possible for the child to return home safety? Indicate any placement restrictions: Section 7: Child Behavior and Characteristics (Historical and Current) This section is a child behavior and characteristic checklist. We recommend that the characteristics match SACWIS (they currently do not). We recommend that the ICCA document allow the user to pull already completed characteristics into the ICCA and also allow that if a new characteristic needs to be entered that is can be directly entered in SACWIS thru the ICCA. We recommend that the date the characteristic was entered and end dated (if applicable) be pulled, as well as, the narrative that is entered. Section 8: Services to be provided to the child and the caregiver We recommend this section be completely revised and the information be directly pulled from the case services section of SACWIS. This information is already entered into SACWIS and should not have to be re-entered. If the services are not yet in SACWIS then we recommend an expandable section to add needed services. Section 9: Special needs of the child All of these items could be listed as characteristics and we recommend that the ICCA document allow the user to pull already completed characteristics into the ICCA and also allow that if a new characteristic needs to be entered that is can be directly entered in SACWIS thru the ICCA. We recommend that the date the characteristic was entered and end dated (if applicable) be pulled, as well as, the narrative that is entered. Section 10: Visitation agreement and arrangements We recommend this section be revised and that the visitation plan be able to be pulled directly from SACWIS. If a visitation plan has not yet been completed we recommend an expandable section where it can be entered. We would request this information then be pulled into the visitation section of SACWIS to avoid duplicate work. Section 11: Educational Information Part of this section states “Provide a copy to the PCSA of the county in which the child is being placed (if child is being placed in “a county other than the county in which the child resided at the time he/she was removed from his/her home.” We request that this be removed because it is not in rule and the other county would not have a need for it. Section 17: Addendums and related forms We recommend removing this section as it asks staff to initial checkboxes that they gave pages (that are part of the ICCA document) to the caregiver. We feel this is redundant since the worker is also signing the ICCA. Section 18: Signatures Recommend adding a 3rd agency representative signature line School Enrollment and Notification Form The form states “This information must be provided to the local PCSA where the child is placed.” This is not in rule and there is not a reason for the local PCSA to have this information. We would also request that this form prepopulate with known information (that has been entered into SACWIS) and school district information. Addendum B Written information provided to juvenile court representative and PSCA in the county of placement Again there is no requirement in OAC for this information to be provided to a PSCA and we would like this requirement to be removed as this is not necessary. In addition, we are required to add safety and wellbeing concerns in regards to the child and the community in this section and it is not listed on this form. We request that this be added. Page 15 The last two boxes on page 15 are not needed. This information should be identified in the case services section. OAC required ICCA information that is missing from this form: We recommend adding religion to the form. This is required by the OAC and not listed. We recommend adding race/ethnicity/ to the form. This is required by the OAC and not listed. We also recommend adding ICWA to the form. This is required by the OAC and not listed.
